IMAT Preparation: How to Win a Place at an English-Taught Italian Medical School

The reason the IMAT attracts applicants worldwide is straightforward. Tuition at Italian state universities is means-tested and typically a fraction of UK, Irish or US fees, teaching is in English, and the degree is recognised across the EU and by many regulators beyond it. The trade-off is that entry is decided almost entirely by one exam score.

The paper is built from four areas: reading comprehension and general knowledge, logical reasoning and problem solving, biology, and chemistry and physics combined. Biology carries the heaviest weight, and the science content sits at roughly upper-secondary level - broad, factual and unforgiving of gaps rather than conceptually deep.

Scoring is what surprises most candidates. A correct answer earns 1.5 marks, an incorrect answer loses 0.4, and a blank answer scores zero. That asymmetry makes disciplined omission a real skill: if you cannot eliminate at least two options, leaving the item blank is usually the higher-expectation choice. Practising that judgement is as important as revising cell biology.

Because seats are allocated by national ranking, your target is relative, not absolute. Cut-offs move each year with the number of places and the strength of the field, and they differ between non-EU and EU-equivalent quotas at each university. Ask two questions before choosing your target: which universities am I eligible to rank for, and what did those cut-offs look like across the last three cycles?

Six months is a comfortable runway for a school-leaver studying alongside coursework. Months one and two rebuild biology from cell structure through genetics, physiology and ecology. Month three covers chemistry - stoichiometry, bonding, acids and bases, organic functional groups - and the small but predictable physics syllabus. Month four is logical reasoning and reading practice, which improves faster with technique drills than with content study.

Months five and six should look like the exam. Sit full past papers under timed conditions, because the IMAT punishes poor pacing more than thin knowledge. After each paper, classify every lost mark as a knowledge gap, a misread, a timing casualty or a bad-risk guess. Those four categories need four different fixes, and treating them all as "revise more" is why plateaus happen.

Past papers are the single best resource available, and there are many years of them. Work through them chronologically and keep a one-page error log per paper. Candidates who review rigorously with three papers outperform those who race through ten.

The application runs in parallel and has its own deadlines: pre-enrolment through the Universitaly portal, choosing your test venue, ranking your university preferences, and later document legalisation and declaration of value for your school qualifications. Missing a portal deadline disqualifies a strong score, so diarise them the moment they are published.
